That sounds reasonably rare. Questions:

- are you talking about the listview icons (on the left side) or the 3D view ones?
- do you wait a sufficient time to see if they come in (they're fetched asynchronously and sometimes take a while to be loaded)?
- a screenshot would be useful
- what memory/gfx/gfx-driver configuration do you have?
